Improvement A3 and Value Stream Summary Template

Purpose. Used by a team to record a structured improvement on one page: the problem, the current state, the analysis, the target state, the actions and the results.
1.When to use this template
Use this template for any improvement that follows a structured problem-solving approach, including value stream mapping events, 5S events, changeover improvements and repeat loss investigations. One template per improvement. The completed template is displayed on the area visual board and filed with the improvement register entry.
2.Current state
Describe the process as it actually runs, with measured data: cycle times, lead time, waiting, inventory, defects, contamination or downtime. Attach the current state map where a value stream or process map was drawn. Show where the waste is with the seven wastes as a checklist.

3.Analysis
Record the root cause analysis using five whys, a cause and effect diagram or another structured tool. Show the evidence for the root cause, not only the opinion of the team.
4.Target state and actions
Describe the target process and the measurable target. List the actions with owners and due dates. Identify any change to standard work, training or the visual board.
5.Results and sustainment
Record the measured result against the target after the actions are complete, the benefit in dollars or tonnes where it can be calculated, the new standard work version and how the improvement will be audited.
