Purpose. Records every sample collected on a monitoring run and every transfer of custody from the field to the laboratory.

1.Purpose of the form

The chain of custody form proves that a sample reported by the laboratory is the sample collected at the monitoring point, at the time stated, and that it was handled correctly on the way. Without a complete form the result cannot be used for licence compliance or client reporting.

2.Completing the form in the field

Complete the header before the first sample and add each sample as it is collected, not at the end of the run. Use the sample identifier from the sampling plan. Record the matrix, the containers, the preservative and the analyses requested. Quality control samples must be listed with their blind identifiers.

3.Transfers

Every person who takes possession of the esky must sign, print their name and record the date and time. Where samples are couriered, the courier consignment number must be recorded and the esky must be sealed with a numbered seal.

4.At the laboratory

The laboratory will record the esky temperature on receipt and any discrepancy between the form and the containers. Discrepancies must be resolved with the technician the same day and recorded in the sample and monitoring register.

5.Retention

A copy of the completed form must be filed with the field sheets for the run and retained for the period in the records procedure. The register entry must reference the form number.

6.Complete the form
